PERMISSION TO USE AMA PROPRIETARY/COPYRIGHTED MATERIALS

The American Medical Association ("AMA") and James J. James, MD, DrPH, MHA ("Trustee") hereby provide David Lakey, MD ("User") the following conditional, non-exclusive, non-assignable, royalty-free, one-time right to reproduce and distribute in print together with the right to record and distribute in electronic media including audio and video tape and to include on User's web-site for one-year the following material:

The images and Power Point slides (Attachment A) that have been used in the Basic Disaster Life Support Provider Manual, Version 2.5, Core Disaster Life Support Provider Manual, Versions 1.01 and 2.0; Basic Disaster Life Support Presentation CD, Version 2.5, and Core Disaster Life Support Presentations CD, Versions 1.01 and 2.5.

(collectively the "Documents") on the basis that:
  1. the AMA retains all ownership interests in the Document(s) including copyright unless otherwise specified in the Document(s);
  2. usage of the Document(s) will be restricted to educational purposes only (and, if applicable, for distribution to participants at the event at which the Trustee presented the Document(s));
  3. no changes, deletions, translations or additions (including the addition of labels, logos or other types of identification) will be made to the Document(s);
  4. no blurring, distortion or alteration will be made to the Trustee's voice or image in the Document(s);
  5. any reproduction of the Document(s) shall include a readily apparent credit line reading as follows: "©2003 American Medical Association. All Rights Reserved." together with all additional copyright\attribution notices appearing within the text of the Document(s);
  6. the Document(s) may not be sold, licensed or used as a supplement to advertising or promotion, either by attachment or enclosure, and any charge for distribution of the Document(s) shall not exceed User's cost of reproduction;
  7. the name, initials or logo of the AMA may not be used except as is authorized herein or otherwise authorized in writing by the AMA; and
  8. if requested by the AMA or the Trustee, User shall provide to the AMA (515 N. State Street, Chicago, IL 60610; attention Officer Services) a copy of each Document(s) reproduced.
