New Contract Clinical Instructors are required to complete the following:

Complete all forms in the packet prior to visiting the Initialization Office.
  1. Personal Information Survey
  2. Standards And Regulations For University Employment
  3. Drug-Free Workplace Policy Statement
  4. Political Aid. Legislative Influence, and Standards Of Conduct Provisions of the 1998-1999 Appropriations Act
  5. U.S. Department of Justice, Employment Eligibility Verification Form (I-9 Form)
  6. List of Acceptable Documents
  7. W-4 Form - Bring Birth Certificate or Passport for verification
  8. Employment Health Questionnaire/TB Test – All Employees Are Required To Have A Baseline TB Test
  9. Limited Power of Attorney and Authorization For Direct Deposit
  10. Verification of Information Affidavit
  11. Authorization for Payroll Deduction for Out-Of-Pocket Insurance Cost
  12. Teacher Retirement System of Texas /Optional Retirement Program Confirmation of Prior Participation

Contract Clinical Instructor must provide and/or complete the following:
  1. Current Texas RN License for verification
  2. Electronic Curriculum Vitae – email to jacarlso@utmb.edu
  3. Provide a copy of academic transcripts for each university degree and request official copies from the universities mailed to:

  4. Current CPR for Health Care Providers for Adult and Child
  5. Complete the Information Resources Security Acknowledgement and Nondisclosure Agreement at http://www.utmb.edu/is/New_IS/Forms_Library/NonDisclosureUTMB.pdf. For additional information contact Patricia Riley at (409) 772-8411 or email to priley@utmb.edu
Contract Clinical Instructors should contact the Academic Program's Administrative Associate at (409) 747-1551 for additional information concerning the following:
  1. SON server access – user name and password
  2. Access to office
  3. Pager – must receive approval from Program Director

Visit Initialization Office (Admin Building - 2.208) -

All new employees must successfully complete their visit to the Initialization Office BEFORE they begin working; or no later than the 3rd day.

             8:00 a.m. - 12:00 p.m.

Parking is available in the garage located at the corner of Market and University Blvd. You are responsible for parking fees.

 

WHAT TO BRING WITH YOU…

If you do not bring these items with you to the Initialization Office, your paperwork will NOT be processed:

ALL names on ALL documents must match exactly, including the name on your professional license RN, MD, RPH ….
  • Completed New Employee Packet (Adobe PDF)
  • Photo ID (for ID Badge Issuance)
  • Application/Resume/or CV (Curriculum Vitae)
  • Social Security Card (for payroll verification) US Social Security Office is located at:

New Employees:
The hiring process cannot be completed until you visit the Initialization Office. Upon completion of Initialization, you will receive final clearance to go to work, including:

  • General Compliance

  • EEO / Diversity / Sexual Harassment
There are additional on-line training courses that you will need to complete as licensed personnel. Compliance training web site:. http://www.utmb-online.com

All required training must be completed within the first 30 days of employment. All required training can be done on-line.

The first time you login to the compliance site you will need to use your UTMB badge number for your User ID and Password. Once you have accessed the courses you will be prompted to change your password to something else.

Should you have problems accessing the web site contact the UTMB Help Desk at (409) 772.5200 ( Galveston) or (713) 513-3050 (Houston) or (888) 898-2401 (outside the Houston/Galveston area.) Should you have questions about the training requirements, contact the Compliance Office at 409-747-8707.

*Training is required every year.
